Here is the scenario when the patient with insurance visits the doctor.

As you can see, now there is more overhead work for the doctor. Doctor now has two functions.  He treats patients and has to file claims and deal with the insurance company.

To alleviate this problem, the doctor hires more employees in his office (in-house) to deal with the insurance company.

Having an in-house staff introduces overheads (extra costs) for the doctor. New employees require salary & benefits ($$$$), physical space, training, computers.  In additions there are HR Hassles, attrition costs, staff not up-to-date with current changes

This is why many doctors choose to outsource their work to medical billing companies.

Medical billing companies assumes the role of in-house staff but is transparent to the patient and insurance company.
